NHS East Midlands is keen to support the personalisation agenda and in particular the implementation of LTC Personalised Care Planning and
Personal Health Budgets across the health and social care community in
the East Midlands. Contact with many organisations has informed us that
there would be great interest in an event to clarify the direction of
travel, link this to the Quality Innovation Productivity and Prevention
(QIPP) agenda, and enable networking and opportunities to share best
practise. We will therefore be holding a Personalised Care Planning,
Personal Health Budget QIPP event and you are formally invited to
attend this event.
Lunch and refreshments will be provided
The event is open to regional colleagues from the third sector, Directors and Commissioners of Social Care, Directors and Commissioners
of PCT’s (City and County), and LTC and PCP leads from across the East
Midlands.
Speakers at this event will include Dr Kathy McLean (Medical Director and SHA QIPP lead) Lorraine Wright (Assistant Director of
Programme Planning EMSHA) and representatives from health and social
care who will showcase their progress thus far and will include a
patient’s own health care experience with Personalised Care Planning.
This event will be supported by representatives from Telehealth companies who will demonstrate their products during the day, and by members of the DH LTC delivery support team.
