Very little is learned from serious case reviews, Baby Peter, Victoria Columbie, Jasmine Bexford and many other Children that have needlessly died over the years, due to Local Authority failings, requiring serious case reviews and public inquiries, costing millions of pounds at the taxpayers expense.
Yesterday we heard that Social Work assistants were left to carry out initial and core assessments, due to the financial cutbacks and restraints put on Local Authorities.
The former is a disaster just waiting to happen; there will be many more Baby Peter like tragedies if unqualified staff are left to carry out work that they are not trained to do.
