The Mental Health register should include patients with severe and enduring mental illness who have consented to be on the register. This could include patients with major psychoses such as schizophrenia and bipolar illness, but it is up to the practices whether to include patients with other long term conditions such as depression, personality disorder or anxiety. This is a prospective code which should be added to the record of those patients that the practice feels fall into this category.

Patients on the Mental Health register should have a review of their physical health, prescribed medication and co-ordination arrangements with secondary care on a annual basis.

Patients on Lithium therapy should have their levels checked 6 monthly and a record of serum creatinine and TSH levels annually.
