With reference to today’s mental health review, could you please inform me how you got the disclosure, of my mental health problems,and who, and what Department this information is shared with?

Due to the present lack of information and transparency, about what Department you work for, i am requesting under section 7 of the data protection act 1998, copies of all files and data, held on computer,and other information,you may have?

You have a mandatory 40 days to comply with my request.

If this information is not provided, within the allocated time scale, i will approach the information commissioner.

This letter was written within 30 minutes of the review, and is not my usual level of competence, due to my stress levels, she says that she has nothing on record, but was writing notes, i will be writing a letter of complaint to my GP over the weekend, to ensure that she deals with the DP request, and also requesting information from him about the register, what it entails , and how to get my name off the register.
